Explanations

•    Dang Shen, Mai Dong and Wu Wei Zi form the formula called Sheng Mai San, which is able to nourish the Yin and tonify the Qi of the Heart.

•    Shi Xiao San contains two substances, Pu Huang and Wu Ling Zhi. They can promote the Blood circulation, dissolve stagnant Blood and stop the pain.

•    Sheng Di Huang, Bai He and Bai Zi Ren can nourish the Heart-Yin and calm the Mind.

Thirty minutes after the formula was given, the pain was much relieved. After the second dose of the herbs 4 hours later, the pain disappeared completely. This formula was continued for another 10 days, after which the ECG had much improved. The patient recovered and was discharged from the hospital after a further 2 weeks. He continued to take this formula but without Pu Hu and Wu Ling Zhi once a day for 2 months. At a follow-up consultation 6 months later, he said he had experienced no further chest pain.

DEFICIENCY OF YIN OF HEART AND KIDNEY

Symptoms and signs

Chest pain with a stabbing sensation, palpitations, night sweating, irritability, insomnia, weakness of the back and knees, dizziness, tinnitus, dry eyes, a red tongue with purple spots and a thready, choppy and irregular pulse.


This syndrome can be seen in coronary artery disease, angina pectoris, menopausal syndrome, myocarditis and pulmonary tuberculosis.

Explanations

•    Sheng Di Huang is sweet and Cold in nature, entering the Heart and Kidney channels. It can nourish the Yin, clear Heat and cool the Blood.

•    Gou Qi Zi and Dang Gui are able to tonify the Blood and moisten Dryness.

•    Sha Shen and Mai Dong both can nourish the Yin in the Upper Burner so as to provide a better condition of Qi and Blood circulation.

•    Chuan Lian Zi is bitter and Cold in nature, and can drain Liver-Fire and unblock the obstruction of Qi.

•    Nu Zhen Zi and Han Lian Cao nourish the Kidney-Yin and reduce Deficient-Fire, but do not have the greasy nature of many Yin-tonifying herbs.

•    Dan Shen, Dan Pi and Yi Mu Cao clear Heat and promote the Blood circulation so as to stop pain.

•    Chuan Niu Xi can cause the Blood to descend.

•    Mu Li can descend Liver-Yang, stabilise the Body Fluids and stop night sweating.

Modifications

1.    If there is severe pain, add Chen Xiang Lignum Aquilariae Resinatum 6 g and Chuan Xiong Rhizoma Ligustici Chuanxiong 9 g to regulate the Qi and Blood so as to relieve the pain.

2.    If there is severe night sweating, add Zhi Mu Rhizoma Anemarrhenae 9 g, Huang Bai Cortex Phcllodcndri 9 g and Di Gu Pi Cortex Lycii Radicis 9 g to nourish the Yin and reduce Deficient-Fire.

3.    If there are severe palpitations and insomnia, add Suan Zao Ren Semen Ziziphi Spinosae 9 g and Bai Zi Ren Semen Biotae 9 g to calm the Mind and improve the sleep.

4.    If there is tinnitus with a high-toned noise, add Shi Jue Ming Concha Haliotidis 15 g and Gou Teng Ramulus Uncariae cum Uneis 9 g to cause the Liver-Yang to descend and reduce Liver-Fire.

5.    If there is Qi deficiency, add Dang Shen Radix Codonopsis Pilosulae 9 g and Zhi Gan Cao Radix Glycyrrhizae Praeparata 6 g to activate the Spleen and tonify the Qi.

ACUPUNCTURE TREATMENT

Explanations

•    PC-7, the Stream point, reduces Heart-Fire.

•    KI-3, the Source point, tonifies the Kidney-Yin so as to nourish the Heart-Yin.

•    HT-7, the Stream point, clears Heart-Heat, calms the Mind and treats restlessness and palpitations.

•    PC-8, the Spring point, clears Deficient-Heat from the Heart and treats a warm sensation in the chest.

•    LR-3, the Source point, regulates the Liver-Qi.

•    PC-6 and CV-17 promote the Qi circulation so as to regulate the Blood circulation in the chest and stop the pain.

•    BL-15 and CV-14, the Back Transporting point and Front Collecting point of the Heart, regulate the Heart-Qi and stop chest pain.

•    SP-6 promotes the Blood circulation and tonifies the Yin and Blood.

Modifications

1.    If there is dizziness and irritability due to hyperactivity of the Liver-Yang, add GB-20 and LR-2 to calm the Liver and suppress the Liver-Yang.

2.    If there is night sweating, add SI-3 and HT-6 to reduce the sweating.

3.    If there is insomnia, palpitations and restlessness, add BL-44, BL-47 and BL-49 to harmonise and tonify the Heart, Liver and Spleen.

Case history

A 52-year-old woman complained of chest pain and pressure in the chest. The pain was triggered or worsened by stressful situations. She suffered also from dry eyes, reduced vision and general tiredness. Her menstruation had been irregular for a year, and she often had migraine after menstruation. She had a purple-red tongue with a thin coating and a deep, thready and choppy pulse.

Diagnosis

Deficiency of Ym and Blood and stagnation of Liver-Qi. Principle of treatment

Explanations

•    The formula Xiao Yao San can smooth the Liver, promote the Liver-Qi circulation, nourish the Blood and strengthen the Spleen.

•    Sheng Di Huang and Dan Shen can clear the Heat in the chest, nourish the Yin, promote the Blood circulation and stop the pain.

•    Gou Qi Zi nourishes the Blood and Ym, and together with Ju Hua benefits the eyes.

•    Xiang Fu can circulate the Qi in the Triple Burner channel so as to regulate the Qi circulation in the chest and stop the pain.

•    Yan Hu Sho arrests the pain.

After herbal treatment for 20 days, the pain had gone. She used the herbs continuously for another 6 months and the other symptoms disappeared too.

DEFICIENCY OF YANG OF THE KIDNEY AND HEART

Symptoms and signs

Chest pain, purple lips and nails, shortness of breath, palpitations, tiredness, an aversion to cold, weakness of the back and cold of the limbs, oedema, a purplish-pale tongue and a deep, weak and irregular pulse.

This syndrome can be seen in angina pectoris, myocardial infarction, heart failure, rheumatic heart diseases, pulmonary heart disease and chronic bronchitis.

Principle of treatment

Tonify the Yang, warm the interior, promote the Blood circulation and stop the pain.

Explanations

•    Rou Gui and Fu Zi are very pungent and warm, strengthening the Yang and warming the interior. Shu Di Huang can tonify the Kidney-Essence so as to provide a substantial foundation for the Kidney-Yang.

•    Shan Zhu Yu and Shan Yao tonify and stabilise the Essence of the Liver and Spleen respectively.

•    Ze Xie can eliminate turbid Damp from the Lower Burner so as to assist Shu Di Huang to tonify the Kidney-Essence.

•    Mu Dan Pi can disperse and clear the restrained Heat from the Liver so as to assist Shan Zhu Yu to stabilise the Liver-Yin.

•    Fu Ling dissolves Damp in the Middle Burner and assists Shan Yao to strengthen the Spleen-Qi.

•    Gan Jiang warms the Middle Burner, and assists Rou Gui and Fu Zi to strengthen the Kidney-Yang and Heart-Yang.

•    Ren Shen and Bai Zhu tonify the Spleen-Qi so as to control the Water ascending from the Lower Burner.

•    Chen Xiang can cause the Qi to descend and stop the pain.

•    Che Qian Zi can increase urination and leach out Damp.

•    When the Heart-Yang becomes stronger, it can disperse the Warmth in the chest and improve the Blood circulation, so chest pain is reduced. When the Kidney-Yang becomes stronger, it can accelerate the water metabolism and also support the Heart-Yang so as to maintain the balance etween Yin and Yang, and between the Upper and the Lower Burners.

Modifications

1.    If there is severe pain and sweating, use Su He Xiang Wan Styrax Pill (I pill a day) in the mean time to promote the Qi circulation in the chest and stop the pain; use Sheng Long Gu Os Draconis 15 g, Sheng Mu Li Concha Ostreae 15 g, Zhi Gan Cao Radix Glycyrrhizae Praeparata 6 g and Wu Wei Zi Fructus Schisandrae 6 g to stop the sweating.

2.    If there is restlessness and insomnia, add Yuan Zhi Radix Polygalac 6 g and Suan Zao Ren Semen Ziziphi Spinosae 9 g to calm the Mind and improve sleep.

3.    If there is severe Blood stagnation, add Chuan Xiong Rhizoma Ligustici Chuanxiong 9 g, Hong Hua Flos Carthami 9 g and Tao Ren Semen Persicae 9 g to invigorate the Blood circulation and eliminate Blood stasis.

4.    If there is severe oedema, add Huang Qi Radix Astragali seu Hedysari 12 g, Gui Zhi Ramulus Cimiamomi 9 g, together with Bai Zhu Rhizoma Atractylodis Macrocephalae 12 g and Fu Ling Poria 15 g to strengthen the Spleen and promote Water metabolism.
