severity class 4 having all ten codes. To examine the three disease staging levels, we use Tables 5-7.
Here, too, patients have a higher probability of a higher category in length of stay as the number of
diagnosis codes changes from 1 to 10; 62% of the patients in level 5 have 10 codes.
While sicker patients will have more diagnosis codes, it is also true that those who are “gamed” will
have a higher number of codes. Moreover, if there is a strong relationship between the number of codes
and the level of severity, any hospital can start increasing the amount of coding in order to shift patients
into a higher category.
Table 8 gives the relationship between the Charlson Index and the number of diagnosis codes. The
triangular nature of the matrix in Table 8 shows that the Charlson Index increases as the number of
diagnosis codes increases.
