ACUPUNCTURE TREATMENT

GV-I Changqiang, BL-25 Dachangshu, BL-57 Chengshan, BL-58 Feiyang, SP-9 Yinlingquan, ST-40 Fenglong and LI-4 Hegu. Reducing method is used on these points.

Explanations

•    GV-I, the Connecting point of the Governing Vessel, specially harmonises the collateral and relieves the anal pain.

•    BL-25, the Back Transporting point of the Large Intestine, and LI-4, the Source point of the Large Intestine channel, clear Toxic Heat in the Large Intestine and promote defecation.

•    SP-9, the Sea point of the Spleen channel, and ST-40, the Connecting point of the Stomach channel, clear Heat and relieve the anal pain.

•    BL-57 and BL-58, the Connecting point of the Bladder channel, harmonise the collateral, eliminate Damp-Heat and relieve the anal pain.

Modifications

1.    If there is anal bleeding, add SP-6 and LI-Il to clear Heat, cool the Blood and stop the bleeding.

2.    If there is redness and swelling of the anus, add BL-25 and BL-35 to clear Damp-Heat in the Large Intestine and reduce the swelling.

3.    If there is constipation, add ST-25 and ST-37 to promote defecation and relieve the constipation.

DEFICIENCY OF QI AND BLOOD

Symptoms and signs

Prolonged persistence of a slight pain in the anus, with a bearing-down sensation in the anus, anal prolapse, lassitude, a pale complexion, dizziness, a poor appetite, palpitations, loose stools, a pale tongue with tooth marks and a white coating and a weak and deep pulse.


Principle of treatment

Activate the Spleen, tonify the Spleen-Qi and relieve the pain.

Tonifying the Middle and Benefiting Qi Decoction

Huang Qi Radix Astragali sen Hedysari 20 g

Gan Cao Radix Glycyrrhizae 5 g

Ren Shen Radix Ginseng 10 g

Dang Gui Radix Angelicae Sinensis 10 g

Ju Pi Pcricarpium Citri Reticulatae 6 g

Sheng Ma Rhizoma Cimicifugae 3 g

Chai Hu Radix Bupleuri 3 g

Explanations

•    Ren Shen, Huang Qi, Bai Zhu and Gan Cao strengthen the Spleen and tonify the Qi of the Spleen and Stomach.

•    Sheng Ma and Chai Hu raise the Yang-Qi.

•    Dang Gui aids Chai Hu to regulate the Liver-Qi and Liver-Blood.

•    ]u Pi promotes the Qi and strengthens the Spleen and Stomach.

Modifications

1.    If there is blood in the stools, add Bai Ji Rhizoma Bletillae 10 g, San Qi Radix Notoginseng 10 g and Xue Yu Tan Crinis Carbonisatus 10 g to stop the bleeding.

2.    If there is prolapse of the anus, add Qing Pi Pericarpium Citri Reticulatae Viride 10 g and Zhi Qiao Fructus Aurantii 10 g to regulate the circulation of Qi and to lift the anus.

3.    If the appetite is poor, add Sha Ren Fructus Amomi 6 g, Mu Xiang Radix Aucklandiae 10 g and Pei Lan Herba Eupatori! 10 g to promote the Spleen-Qi in order to improve the appetite.

ACUPUNCTURE TREATMENT

GV-I Changqiang, GV-4 Mingmen, GV-20 Baihui, BL-25

Dachangshu and BL-23 Shenshu. Reinforcing method is used on these points. Moxibustion treatment is recommended.

Explanations

•    GV-I, the Connecting point of the Governing Vessel, harmonises the collateral and relieves the anal pain.

•    BL-25, the Back Transporting point of the Large Intestine, regulates the Large Intestine and relieves the anal pain.

•    GV-20, the crossing point of the Governing Vessel and the Bladder channel, promotes the function of Yang-Qi to lift the Internal organs.

•    GV-4 tonifies the Yang-Qi.

•    BL-23, the Back Transporting point of the Kidney, tonifies the Kidney and warms the Yang-Qi in order to dispel Internal Cold.

Modifications

1.    If the appetite is poor, add ST-36 and CV-12 to tonify the Spleen-Qi and improve the appetite.

2.    If there is anal prolapse, add CV-4 to tonify and raise the Yang-Qi and to relieve the prolapse.

3.    If there is constipation, add ST-25 and ST-37 to promote defecation and relieve the constipation.

Case history

A 56-year-old woman had been suffering from haemorrhoids for 24 years. She complained about anal pain, which had worsened 6 months before after a haemorrhoid operation, and about a prolapse of the anus. She looked pale, had a poor appetite, dizziness, fatigue, palpitations, constipation, a pale tongue and a weak pulse.

Diagnosis

Deficiency of Qi and Blood.

Principle of treatment

Tonify Qi and Blood to lift the Internal organs.

Acupuncture treatment

GV-1, GV-4, BL-20, BL-23 and BL-25 were selected. Reinforcing method was used. Treatment was given once every other day. Besides this, the patient was asked to warm CV-12 and CV-14 with moxa daily for 15 minutes.

Explanations

• GV-I is the Connecting point of the Governing Vessel and the patent point for treating anal disorders.

•    GV-4 tonifies the Qi and Blood to treat haemorrhoids with ana) prolapse.

•    BL-20 is the Back Transporting point of the Spleen, and BL-23 is the Back Transporting point of the Kidney. These two points in combination tonify the Qi and Blood.

•    BL-25, the Back Transporting point of the Large Intestine, relieves the anal pain.

•    Moxibustion promotes the Yang-Qi and supports the internal organs in order to relieve the prolapse.

After 2 months the pain and prolapse were under control. Then Bu Zhong Yi Qi Wan Tonifying the Middle and Benefiting Qi Pill was prescribed to stabilise the treatment effect. Upon follow-up 2 years later she reported being free of the prolapse and pain since the treatment.

STAGNATION OF QI AND BLOOD WITH FORMATION OF HEAT IN THE BLOOD

Symptoms and signs

Prolonged anal pain with a fixed location, or stabbing pain with a hot sensation, which is aggravated by pressure, defecation and when walking, thirst, swelling and redness of the anus, constipation, restlessness, a purplish tongue and a wiry or choppy pulse.

Principle of treatment

Circulate Qi and Blood, clear Heat, remove Blood stasis and sedate the pain.

Explanations

• Zhi Qiao and Jing Jie regulate the circulation of Qi.

•    Tian Hua Fen, Sheng Ma, Sheng Di Huang, Huang Lian and Huang Qin clear Heat in the Blood.

•    Dan Shen, Dang Gui and Chi Shao cool the Blood and remove Blood stagnation.

•    Di Yu, Huai Jiao and Wu Ling Zhi remove Blood stagnation and stop the bleeding.

Modifications

1.    If there is swelling and redness of the anus, wash the anus with a lotion obtained from Ku Shen Radix Sophorae Flavescentis 10 g, and Mang Xiao Natrii Sulfas 10 g to reduce the swelling and relieve the redness.

2.    If there is constipation, add Da Huang Radix et Rhizoma Rhei 10 g, Mang Xiao Natrii Sulfas 10 g to promote defecation and relieve the anal pain.

3.    If there is restlessness, add Long Dan Cao Radix Gentianae 10 g and Xuan Shen Radix Scrophulariae 10 g to clear Heat and relieve the restlessness.

Explanations

•    GV-1, the Connecting point of the Governing Vessel, regulates the circulation of Qi and harmonises the collateral in order to relieve the anal pain.

•    BL-17, the Gathering point for the Blood, promotes the circulation of Blood and removes its stasis.

•    BL-18, the Back Transporting point of the Liver, regulates the circulation of Qi and Blood and removes Blood stagnation.

•    LI-4, the Source point of the Large Intestine channel, clears Intestinal Heat and promotes defecation, as well as relieving the anal pain.

•    BL-57 and BL-58, the Connecting point of the Bladder channel, harmonise the collateral, regulate the circulation of Qi in the Large Intestine and relieve the anal pain.

Modifications

I.  If there is a sharp or stabbing pain around the anus, add LU-6 and LI-7, both Accumulation points, to promote the circulation of Blood in the Large Intestine and relieve the pain around the anus.

2.    If the pain worsens at night owing to severe stagnation of Blood, add BL-30 and BL-36 to regulate the circulation of Blood and relieve the anal pain.

3.    If there is pain caused by constipation, add SP-6 and KI-6 to promote the production of Body Ruids and relieve the constipation.

Case history

A 36-year-old man had been diagnosed as suffering from haemorrhoids and anal fistula 3 years previously. During the last 2 years he also complained about a painful anus. Chinese herbal treatment had given some relief, but for 2 weeks the pain had been unbearable. Prolonged sitting or riding his bicycle caused a sharp pain, and as well as the painful anus he also had a hot sensation in his anus and constipation. His tongue was purplish and his pulse was wiry.

Diagnosis

Blood stagnation with accumulation of Heat in the Large Intestine.

Principle of treatment

Clear Heat, regulate the circulation of Qi and Blood and remove Blood stagnation.

Acupuncture treatment

The points GV-1, BL-17, BL-40 and SP-10 were needled with even method every other day. The blood-letting method was used on BL-40.

Explanations

•    GV-1, the Connecting point of the Governing Vessel and the patent point for anal disorders, harmonises the Collateral and relieves the anal pain.

•    BL-17, the Gathering point for the Blood, regulates the circulation of Blood and removes its stasis.

•    SP-10 clears Heat and eliminates Blood stasis.

•    BL-40, the Sea point of the Bladder channel, relieves anal pain. Bleeding at this point can also clear Heat in the Large Intestine.

After 20 days of treatment, the anal pain was under control. Upon follow-up a year later he reported being free of the pain since the treatment.
