4.2.9 Testing Report Contents
The testing report should describe the test carried out and the results obtained with
reference to the test scope, the assumption and the theoretical framework, the
detailed description of the followed procedure in order to repeat the test, the results
and their analysis.
The testing report structure should be the following:
4.2.9.1 Heading
Title.

The name and address of the laboratory, and the location where the tests and/or
calibrations were carried out, if different from the address of the laboratory.

A unique identi
cation of the test report or calibration certi
cate (such as the
serial number), and on each page an identi
cation so as to ensure that the page is
recognized as a part of the test report certi
cate, and a clear identi
cation of the
end of the test report certi
cate.
The name and address of the customer.

Identi
cation of the method used.
￿
The date of receipt of the test item(s) where this is critical to the validity and
application of the results, and the date(s) of performance of the test.
￿
The name(s), function(s) and signature(s) or equivalent identi
cation of person
￿
(s) authorizing the test report.
4.2.9.2 Summary
A synthesis of the test report that is also comprehensible if separated from the
complete test report. It should introduce the report contents in a clear and succinct
way.
Objective: the de
nition of the test objective and what the test aims to measure
or demonstrate.
4.2.9.3 Theoretical Framework
The description of the theoretical principles, equations and assumptions used during
the experiment or in the results analysis should be listed. The de
nition of the
parameters used, if necessary, should be included in reference to topics, publica-
tions or other bibliography sources (with a complete list of references included in
the appendix).
