were asked to indicate on the questionnaire if they had been diagnosed with
chronic diseases by a physician: asthma, emphysema, Crohn's disease, migraine
headache, ulcerative colitis, chronic bronchitis, irritable bowel syndrome, allergies,
multiple sclerosis, Parkinson's disease, scleroderma, skin disease, poliomyeli-
tis, autism, skin cancer, and, arthritis/osteoarthritis. Lastly, physician diagnosed
acute diseases listed on the questionnaire included: leptospirosis, salmonellosis,
shigellosis, typhoid fever, hepatitis A, poliomyelitis, amoebiasis, bronchitis, pneu-
monia, upper respiratory infection, lower respiratory infection, cold, giardiasis, and
gastroenteritis.
