Basic investigation procedures
This section outlines the basic investigation procedures
for investigation of all types of adverse events and
discusses:
The level of investigation
Involving others
Conducting the investigation
Gathering information
Analysing information
Identifying risk control measures
Agreeing and implementing an action plan.
The level of investigation
As mentioned above the level of resources allocated to
the investigation will depend upon the seriousness of
the actual or potential outcome of the event.
The decision regarding the level of investigation,
particularly for near miss events and undesired circum-
stances, is quite often complex. To aid this process and
