intravenously before induction of heart arrest. Perfusion
fi xation should be started within 5 min after heart arrest
is confi rmed.
(b) Thoracotomy is started under deep anesthesia or deep
analgesia before heart arrest, if your institute allows this
procedure. Artifi cial respiration under intubation is
necessary in order to minimize the infl uence of changes of
arterial blood gas pressure. Perfusion fi xation can be
started under more physiological condition, however, the
motion of heartbeat and scatter of blood might disturb
the operation.
2. Whether the descending aorta should be cramped or not.
The answer is not conclusive. When it is cramped, the volume
of perfusion solution can be saved. However, it is unclear
whether physiological cerebral perfusion pressure can be main-
tained under cramping the descending aorta.
3. What is the optimum perfusion pressure?
The answer is not conclusive. The mean arterial blood pressure
or the systolic arterial blood pressure of the experimental ani-
mal are usually selected. In our previous experiment using dogs
( 5 ), morphological differences cannot be seen between the
dogs perfusion-fi xed under the systolic arterial blood pressure
and those under the mean arterial blood pressure.
