We might have a concern that when we plan a break or even a
night between the toolbox meeting and the actual execution of a task,
the actors might have problems remembering the (safety) details of the
task. For this reason, it is good to refresh all safety-related elements in
a Last Minute Risk Assessment (LMRA) 1 . As the name implies, the
LMRA is done just before the execution of a task, preferably on the
spot. Recalling the safety elements once more ensures they are handled
with priority while doing the task.
A Last Minute Risk Assessment (LMRA) is a final check to see whether
all possible risks are acknowledged and precautions have been taken.

Optimal preparation
The ideal way to prepare for a complex task in which many people are
involved is to first organize a toolbox meeting in which all the ins and outs
of the task are discussed (technical preparation), and then take a break or
an evening off. As a last preparation, all people involved conduct a
LMRA (safety preparation) just before the actual task starts.
Case 1
Any idea what caused the problem in the case study? A problem usu-
ally has more causes that coincide at a certain moment. In this case,
there were three independent problems:
1. The operator was new on this plant and could not contribute to a
toolbox meeting, which was not held because there was no input
other than the knowledge of the two mechanics.
1 A Last Minute Risk Assessment (LMRA) is a meeting on the spot of the action with all the people
who are directly involved in the execution of a task. It is a last check in which all the possible risks
are assessed once more in combination with the possible activities that might be suitable in case an
unforeseen risk might occur. Usually the LMRA is supported by a checklist of, for example, 10
questions to help the people check whether they understand the task, know the safety procedures,
and so on. More practical information about the LMRA is presented at the end of this chapter.
