(Annexure A) at your earliest to the undersigned to be forwarded to the ABC Pharmaceutical
Company QA and regulatory affairs department.
The information is mandatory to raw material and semifinished bulk materials import to the
company.
It is requested to kindly fill Annexures B, C, D, and E mentioned in Annexure A and send the
related documents (originals) at your earliest to the undersigned.
Based on your feedback, the specific documents will be directly requested by our QA and RA
departments as applicable.
