epi or endocardial scarring). Clinical trials have shown better responses with endocar-
dial pacing [

], although this may be due to the limited number of pacing sites that can
be reached due to the coronary anatomy rather than an indication that pacing from the
endocardium or epicardium is intrinsically better.
