1 = Full-time Employee started at the company between 1950 and 1954
2 = Full-time Employee started at the company between 1955 and 1959
3 = Full-time Employee started at the company between 1960 and 1964
4 = Full-time Employee started at the company between 1965 and 1969
5 = Full-time Employee started at the company between 1970 and 1974
6 = Full-time Employee started at the company between 1975 and 1979
7 = Full-time Employee started at the company between 1980 and 1984
8 = Full-time Employee started at the company between 1985 and 1989
9 = Full-time Employee started at the company between 1990 and 1992
A = Full-time Employee started at the company between 1930 and 1970 and left for a
period of longer than six months.
B = Full-time Employee started at the company between 1970 and the present, and
tested positive for a communicable disease or refused the test.
C = Part-time Employee who is a relative of the owners of the company.

ABC = Employee currently has elected the ABC coverage plan for self only.
LNM = Employee currently has elected the LNM coverage plan for self only.
WIN = Employee currently has elected the WIN coverage plan for self only.
KJW = Employee currently has elected the KJW coverage plan for self only.
PRD = Employee currently has elected the PRD coverage plan for self only.
ABX = Employee currently has elected the ABC coverage plan for family.
LNX = Employee currently has elected the LNM coverage plan for family.
WIX = Employee currently has elected the WIN coverage plan for family.
KJX = Employee currently has elected the KJW coverage plan for family or had converted
to KJW coverage (for self and/or family) after maintaining ABC coverage for self
and family or WIN coverage for self or for self and family for at least 12 consecutive
months anytime during the last ten years.
PRX = Employee currently has elected the PRD coverage plan for self OR family type
coverage.

DEN = Denied optional coverage
MED = Failed physical
DIS = Disqualifying pre-existing condition
PRE = Pregnant
