Licenses
Evidence of professional engineer, general contractor, business,
and any special licenses
Noncollusion Affidavit
Typically a standard form provided by owner's legal or
procurement department
Project Team
Description of the project team, the organizational structure,
and the relationships among team members; organization
chart
Team Experience
Documentation of team experience in executing similar projects
(include examples), including projects where members have
worked together in the past
Reference Projects
List reference projects and provide the team's role, project size,
location, cost, owner information, and reference contact
Project Team Member
Experience
This is very important in a qualification selection; individual
resumes of each team member highlighting their experience
on similar projects and their role
Management Approach
Description of the team's management approach to the project
and how the approach will ensure project success
Project Controls and
Systems
Definition of the project control strategy and systems to be used,
illustrating how this will contribute to the success of the project
Financial Capabilities
Materials that provide evidence of proposer's financial strength;
may require parent company guarantee or letter of credit
Minority/Woman/Small
Business Enterprise
(M/W/SBE) Plan
Materials that demonstrate proposer's compliance with owner's
M/W/SBE requirements; state percent participation of each
subcontractor or subconsultant; provide letters of intent for
each subcontractor or subconsultant
Evidence of Required
Bonds
Performance, payment, other
Insurance Forms
Evidence of required insurance and other surety documentation
Supplemental
Information
Additional firm information, plans, nonspecified drawings,
project awards, reference letters, and so on.
