pletion Date
The date that an individual filled in the survey. If there is no date on the survey, this date field defaults
to the date the survey form is received by the application.

The inquiry an organization uses to seek feedback. Questions can be either rating questions, fixed re-
sponse non-rating questions such as Gender, or free-form text questions.
The individual who completes the survey form. We may know little (if anything) about this person other
than that he or she completed a survey form. This person does not have to be a customer of the organiz-
ation to complete a survey.
