in an entire community and help assure they don't get out of control between visits
to their provider. This is a key goal of all chronic care models and technology is now
available to greatly facilitate achieving it. Similarly, popHealth and Wellcentive
allow an entire provider network to know each provider in that network and even
that each patient of each of those providers is being managed against well defined
and accepted quality metrics. This is an essential issue if healthcare quality is to be
improved and, ultimately, if costs are to be reduced because problems aren't allowed
to get worse over time as a result of important screening or clinical management
issues not being routinely addressed.
