H-411 ( Communicable Disease Report Part 1 ) and enters data in her outward
register. The data of all confirmed cases are also entered in the Infectious
Diseases Register (H-700) at the Public Health Inspector Office. The public
health inspector will then return the completed H-411 and H-544 to the
medical officer of health.
At the latter's office, on receiving the H-544 and H-411 forms from the pub-
lic health inspector, the Medical Officer of Health updates the notification
register and then enters data of confirmed cases in the Infectious Diseases
Register (H-700). For each confirmed case the form H-411a is completed using
the data on the form H-411 sent by the Public Health Inspector.
Every week the medical officer of health completes the Weekly Return of
Communicable Disease (WRCD-H-399) based on notifications in the Infectious
Diseases Register. The H-399 and H-411 forms for the particular week are sent
to the Epidemiological Unit in Colombo with copy to the regional epidemiolo-
gist. A third copy is retained in the Medical Officer of Health Office for future
reference. This is the most important activity of the Medical Officer of Health
in the notification system for which she is personally responsible. The officer
