Defect life cycle.
The life cycle of a defect includes open for assignment, assigned, open for veri-
fication, failed, reassigned, passed, or closed (see Figure 7.5).
In the defect tracking system, the tester should open a new defect when he finds
it. He should use a proper template for defect logging so that it will be easier for
everybody in the loop who is responsible for assigning defects, fixing defects, reas-
signing defects, and closing defects.
Here is a template that can be used for reporting defects:

Monitoring of Production Systems
