(change in volume) or distortion (change in shape). The discontinuum creates instability across
the trabecular-cortical boundary as energy dissipation may not be consistent. When a flaw exists
that could be caused by the micro-failure of the trabeculae in daily activities, the discontinuous
energy rapidly dissipates, leading to crack propagation and delamination between the cortical
shell and the trabecular core. Crack propagation along the laminated layer results in the crescent
sign or fracture.
This study reviewed literature associated with clinical and biomedical engineering aspects of
ONFH. A finite element simulation of ONFH was conducted and the associated pathomechanisms
were examined. Further clinical investigations and validation should be carried out to verify these
hypotheses and pathomechanisms.
